`Plaintiff, by counsel, petitions the court and says:
`1.
`Plaintiff owns a judgment obtained in this court against the judgment defendant
`on 02/13/2008 for the sum of $805.64, plus post judgment interest and costs.
`Plaintiff has no cause to believe that execution against the judgment defendant will
`satisfy the judgment.
`3. Garnishee Defendant has or will have in its possession one or more deposit
`accounts as defined by I.C. 28-9-2-1. et seq. in which Defendant(s) has (have) an
`interest.
`WHEREFORE: Plaintiff prays as follows:
`1.
`An order issue requiring the judgment defendant JUAN G GONZALEZ:
`a.
`To appear via telephone at a designated time and to answer as to any non-
`exempt property which can be applied to the satisfaction of said judgment;
`To answer as to wages, assets, profits, income, or other non-exempt
`property which the judgment defendant has, or which from time to time the
`judgment defendant may have due and owing from garnishee defendant.
`An order issue requiring garnishee defendant to appear telephonically or answer
`interrogatories under oath concerning the wages, assets, income, profits, or other
`non-exempt property due, or to become due, to said judgment defendant and return
`same to this court on or before the date set out in said interrogatories.
`The court, after hearing, issue an appropriate order to apply said property towards
`the judgment pursuant to statute.
`I affirm, under the penalty of perjury, that the foregoing representations are true.
